Template

The FSMA 204 records template. Free, in Excel.

A tab for each event produce businesses record, with a column for every key data element, so your records are already in the sortable spreadsheet the FDA can ask for.

Excel workbook · works in Google Sheets and Numbers · no sign-up

What’s inside

  • A “Start here” tab on how to fill it in
  • Harvesting: a column for every key data element, one row per lot
  • Cooling: a column for every key data element, one row per lot
  • Initial packing: a column for every key data element, one row per lot
  • Shipping: a column for every key data element, one row per lot
  • Receiving: a column for every key data element, one row per lot
  • Transformation (repacking, cutting, mixing): a column for every key data element, one row per lot
  • A “Traceability plan” tab with the five things the plan covers

How to use it

Use the tabs for the steps you handle, and add a row each time food on the Food Traceability List is harvested, cooled, first packed, shipped, received or repacked. Keep the lot code exactly as it’s printed. In a recall, sort and filter by lot code to hand over every record for that lot.

A spreadsheet works while volumes are small. When typing rows by hand stops keeping up, Clovero records the same columns as you receive, repack and ship. See how

The template follows the FDA’s rule (21 CFR 1 subpart S) as of October 2026. It’s a starting point, not legal advice.

Rather not type every row?

Clovero fills in these records as your team works. Bring one lot and we’ll show you.
